A Legal Disclaimer
iMed knows the importance of maintaining the integrity of privacy of our patients/members. In the course of providing you with telemedicine services, it is necessary for iMed to gather both personal and medical information. We take all reasonably available steps to safeguard information of a confidential nature. We maintain the privacy of medical and health information of any individual for whom we provide services (“Protected Health Information” or “PHI”) and comply with all relevant state, and federal laws. Some of this information is covered by HIPAA. To address this, we have added provisions to each of our policies that confirm that we will keep the protected information confidential as required by HIPAA. We abide by the terms of this Notice, as amended from time to time. iMed will never voluntarily disclose or use the information except as outlined in this statement.
Governmental Proceedings
We may be required to disclose PHI for the following public health activities:

Information to public health authorities for the purpose of preventing or controlling disease, injury or disability;

Information to a health oversight agency that oversees the health care system and is charged with responsibility for ensuring compliance with the rules of government programs

Child abuse, neglect or domestic violence, to a governmental authority, including a social service or protective services agency, authorized by law to receive reports of such abuse, neglect, or domestic violence;

Information about products and services under the jurisdiction of the U.S. Food and Drug Administration;

To a person who may have been exposed to a communicable disease or may otherwise be at risk of contracting or spreading a disease or condition;

To prevent or lessen a serious and imminent threat to a person’s or the public’s health or safety

To organizations that facilitate organ, eye or tissue procurement, banking or transplantation;

For research purposes if an Institutional Review Board/Privacy Board approves a waiver of authorization for disclosure; and

Information to your employer as required under laws addressing work-related illnesses and injuries or workplace medical surveillance.
Your Right regarding your PHI

Confidential Communication. You have the right to receive confidential communications of your PHI. You must submit your request in writing to iMed at iMedoncall247@gmail.com. We will make all reasonable efforts to accommodate your request.

Restrictions. You have the right to request restrictions on certain uses and disclosures of PHI for treatment, payment or health care operations. You must submit your request in writing to iMed. iMed is not required to comply with your request if it is required by law or in cases of emergency.

Inspection and Copies. You have the right to inspect and copy your PHI. You must submit your request in writing to iMed.

Amendment. You have a right to request that iMed amend your PHI if you believe it is incorrect or incomplete. You must submit your request in writing to iMed and provide a reason to support the requested amendment. iMed may, under certain circumstances, deny your request by sending you a written notice of denial.

Breach Notification. You have the right to be notified in the event that iMed (or an iMed Business Associate) discovers a breach of unsecured PHI.

Third Party Sites
The iMed website content consists of a number of hyperlinks to the sites of third parties. These hyperlinks are provided as a service to our users when the site might contain some pertinent information for our users. The privacy policies of these third-party sites, such as the types of information collected and the use of the collected information, is unknown to and beyond the control of iMed.
